Patient Consents
- One chronic condition expected to last between three months to one year or until the deatil of the patient
- Comprehensive care plan established, implemented, revised, or monitored
- Monitoring must be performed by a Physician, QHCP, RN, or Clinical Staff
REIMBURSEMENTS
99426 – PCM Clinical Staff (Firt 30 Mins) $60 – $70
First 30 minutes of clinical staff time per month directed by a physicial/QHP.
99427 – PCM Clinical Staff (Add-On 30 Mins) $58.62
Each additional 30 minutes of clinical staff care management.
